Through the Centers for Disease Control and Prevention’s Healthy Communities Program, communities are working to reduce chronic diseases by changing the places and organizations that touch people’s lives every day. The program’s tools for community action include:
The Healthy Communities Program has funded 331 communities and 52 state and territorial health departments to build community capacity around reducing health risk factors and support health equity. The program also collaborates with national networks to provide technical support and training to its communities, including the US State and Territorial Health Departments Collaborative, the National Association of County and City Health Officials (NACCHO) and the YMCA of the USA.
